LashLife Policies and Procedures
By booking an appointment online with LashLife you are agreeing to the following:
I understand that the eyelash extension procedure requires synthetic eyelashes extensions to be glued to my own natural lashes.
I understand that it is my responsibility to keep my eyes closed for a duration of approximately 60-120 minutes during the procedure.
I agree to disclose any allergies that I may have to latex, surgical tapes, adhesive, cyanoacrylate, etc.
I agree to disclose any diseases and or disorders including but not limited to claustrophobia.
I understand that I am required to follow the eyelash extension care advised by my eyelash technician in order to maintain the life of my eyelash extensions. Failure to do so may result it premature lash extension loss.
I agree that by reading and agreeing to this consent form, I release LashLife from any claims or damages of any nature.
I agree the I have read and fully understand this consent form.
I give LashLife permission to show my before and after photos of eyelash extensions to other potential clients as well as post the pictures to the company website and any relevant social media accounts. Please advise us prior to service if you do not wish for us to photograph you or your lashes.
I understand that a reaction may occur, and will inform my lash technician if I have any signs of irritation. In addition to seek medical attention under my own expense should a reaction occur. Please note if a reaction does occur the extensions will have to be removed and I will not receive a refund for service.
A credit card is required to reserve all first appointments and the card number will be kept on file for future appointments.
Cancelling and Rescheduling Policies:
LashLife requires a MINIMUM of 24 hours notice for any cancellations or rescheduling of appointments. If you have not provided at least 24 hours notice for a change in appointment, you are required to pay for 50% of the scheduled appointment/appointments costs. This will be billed to the credit card on file unless other arrangements are made.
A email reminder is sent 48 hours prior to all appointments. 24 hours prior to the appointment all appointments are considered confirmed.
All training classes require a deposit that is non refundable and non transferable. If you cancel your training once booked you will forfeit the required deposit. If you need to reschedule training it must be done more than 24 hours prior or 50% of the Training course cost will be charged. No shows for Training courses will be billed 100% of the course cost.
No Show/Late Policy:
Should you be more than 15 minutes late to your appointment you will be required to reschedule your appointment and pay for 50% of your scheduled appointment/appointments. LashLife reserves the right to require you to prepay for future appointments with no refund given if the cancellation/rescheduling policy is not followed for all prepaid appointments.
No shows will be charged 100% of the appointment cost.
Service Return Policy:
We do not give refunds on any services rendered. Due to the nature of the service, please notify LashLife within 72 hours of service date with any problem and/or concerns regarding your service. We will gladly correct the problem within that time frame, as refunds will not be given. If you arrive to your appointment with eye makeup on you will not qualify for the 72 hour guarantee.
Refills for eyelash extensions are required approximately every 2-3 weeks depending on natural lash cycle and proper care. A refill scheduled over 4 weeks from the previous appointment will need to be rescheduled as a full set. Also in order to qualify for a refill you must have a MINIMUM of 40% of your lashes remaining. If you have under 40% lashes remaining a new full set will be charged.
If you require any changes or additional services, these must be requested prior to your appointment. This will allow us to ensure you are scheduled for the appropriate amount of time for your scheduled services. We offer multiple different refill options please ensure you are scheduling enough time for your lash needs as we may not be able to accommodate additional time needed the day of your scheduled appointment.
LashLife's prices are subject to change without notice and are subject to applicable taxes.
Refund Policy for Products:
Products purchased cannot be returned or refunded once they have left the studio for hygienic reasons.
